Step 1

If you want to get your Ayushman card made, first check your eligibility.

Visit the official website of the scheme pmjay.gov.in.

Click on the 'Am I Eligible' option.

Step 2

Then, enter your 10-digit mobile number.

An OTP (One Time Password) will be sent to this registered number.

Enter the received OTP.

Step 3

After that, you will see two options. First, select your state.

To search for others, enter your ration card number and mobile number.

Now you will get information about your eligibility.

Application procedure:

If you are eligible for the scheme, you will have to visit your nearest Jan Seva Kendra for the application process.

Meet the concerned officer there and submit your documents.

After eligibility and document verification, your application is submitted upon successful verification.
