How to apply
     You have to go to the official website of the scheme (https://pmjay.gov.in/).
     Now you have to select the option of Am I Eligible?
     After this, the phone number will have to be verified and OTP generated.
     Now OTP has to be entered.

     After this, entering the state, name, phone number, and ration card number on the new webpage, you can search your eligibility.
     If you are eligible for the scheme then you will have to visit your nearest Jan Seva Kendra.
     Now you have to verify the documents required for the scheme.
     After this, you will have to fill the application form and submit it by attaching the documents to it.
     After a few days you will get the Ayushman card.
